How can I get my medical records transferred to K2 Dermatology?
Go to Forms and Info and download a Release of Medical Records Request Form. Fax the form to the location of the dermatology clinic you would like your records transferred from and follow the instructions of the FAQ above to make an appointment at K2 Dermatology Clinic.
Where is K2 Dermatology located?
K2 Dermatology is located at 3434 47th Street Suite 200, Boulder, CO 80301.
Just east of Foothills Parkway at the corner of the Diagonal Highway and 47th Street in the Northpoint Offices (also known as the Colorado Group Building).
Does K2 Dermatology Clinic accept Medicaid?
At this time K2 Dermatology Clinic does not accept Medicaid.
Can I fill out the forms ahead of time?
Absolutely.

Just download the form onto your computer and print them. Don’t forget to bring the form with you when you come to our office for your appointment (there’s nothing worse than filling out forms twice).
